GP LETTER REQUESTS
Unfortunately we can only provide letters based on factual information contained within your medical record.
GP letter request forms are available from Reception - letter requests are normally processed within 5 working days.
These are private work and will incur a fee.
We DO NOT write housing or benefit support letters as these agencies will contact the practice directly if they require any specific information.
We appreciate your understanding.

This service is NOT FOR EMERGENCIES such as stroke, heart attack, loss of consciousness, breathing difficulties, severe bleeding or major trauma - please call 999 as usual in these instances
